- Every permit shall be granted or received one of the following forms as may be appropriate -

1. in respect of a particular Stage Carriage/service of Stage Carriage in Forms V and VA respectively,

1. in respect of a particular Contract Carriage permit in Form IV,
2. in respect of a Casual Contract Carriage in Form VI,
3. in respect of Contract Carriage for private hire in Form XV,

1. in respect of a Goods Carriage permit in Form X,

1. in respect of a Temporary permit in Form XII,
2. in respect of a Special permit in Form XIV,
3. in respect of a Private Service Vehicle permit in Form XVI.
