It shall be the duty of the mediator to disclose—

1. any personal, professional or financial interest in the outcome of the consumer dispute;
2. the circumstances giving rise to justifiable doubts as to his independence or impartiality; and
3. the facts and circumstances that may give rise to a presumption of bias, at any stage during the mediation.
